7/18/2023 EnchantaVaughn Approved Behavior: Client presented with a normal spirit and affect. Client was in a pleasant mood today and presented good behaviors, as evidence of her smiling and being polite toward her family. Client displayed calm and peaceful expressions, as she waved hello while presenting a respectful tone toward mental health counselor. Behavior: Client was sociable and respectful to her family during the family activity. Client was excited to express her feelings and share her favorite thing she loves about having counseling as a way to help her understand how it keeps her grounded and able to deal with things internally that she didnât know how to do before. Intervention: QMHP went to the home to meet with client to discuss her recent progressions. QMHP engaged in conversation with client about factors that contribute to ADHD. QMHP utilize positive self-talk strategies to help client think positively at all times and to talk about her actions and illness. QMHP utilized active listening skills while client was verbalizing information she learned. QMHP provided simple praises to client for her commitment to her progress. The QMHP demonstrated and provided positive and empathetic acceptance. QMHP reviewed the positive things that client is doing and encouraged her to continue in making positive decisions in the home. Intervention: QMHP gave client the opportunity to express and explore her perceptions of how her symptoms of ADHD can affect her. QMHP continued to monitor clientâs relationship with mother and compliance to duties and expectations for the client in the home and in the community. QMHP encouraged client to use her strategies and skills with peers, family, and relatives to support her with managing symptoms associated with ADHD. QMHP provided a list of experiences that is common with her mental illness. QMHP continued to elaborate on ways client can cope and manage stressors. Response: Client and mom were observed interacting well with one another before giving QMHP their attention. Both client and guardian provided positive feedback, actively participated, and engaged with QMHP. Client was receptive and open to the interventions, as she was focused on the information and activities that were being processed without any difficulties. Response: Client and guardian were acceptive of QMHP interventions. Client appears to be calm and relaxed after the interventions were implemented. Client shared some of the things she does at home to help her relax. Client states that going to dance practice, getting her nails done, getting her hair done all make her feel relaxed. Client presents moderate self-esteem throughout the session, as evidenced by utilizing creative strategies to stay motivated and on task. Client communicated that he felt safe and comfortable talking to the in-home counselor. Client acknowledges expectations and reviews goals and objectives with counselor and parent.
